Caspofungin

Caspofungin is the first approved member of the echinocandins group of antifungal drugs.

Mechanism of action:

Echinocandins interfere with the synthesis of the fungal cell wall by inhibiting the synthesis of B(1,3)-D-glucan, a component of fungal cell wall. This leads to lysis and cell death.

Pharmacokinetics:

  1. Caspofungin is available for administration by intravenous route only. Oral route administration is not available.
  2. The dosage is 70 mg intravenously on day 1 then followed by 50 mg/day
  3. Caspofungin is highly bound to serum proteins and has a half-life of 9 to 11 hours.
  4. The drug is slowly metabolized in liver by hydrolysis and N-acetylation.

Spectrum of  susceptible organisms :

Efficacy of capsofungin is limited to Aspergillus and Candida species only.

Side effects include fever, rash, nausea, and phlebitis.
